Blue Cross Complete of Michigan sought to address food instability as an unmet social determinant of health (SDOH) and help members better manage chronic conditions and avoid hospitalizations and readmissions through implementing a program of fully prepared home-delivered meals. An analysis of member claims data 9 months and 12 months after the meals intervention demonstrated reductions in emergency department and in-patient utilization, length of stay, and cost of care. Medication spend increased, indicating improved adherence.

We'll discuss our claims analysis 9 and 12 months after an intervention of home-delivered meals providing evidence for a lasting impact of a shorter term (average 45 meal) nutritional intervention.
