Today, the majority of U.S. adults live with a chronic condition. 6 in 10 U.S. adults are living with a chronic condition but chronic conditions do not exist in isolation and 4 in 10 people are living with two or more. Those living with chronic condition(s) interact with the health care system in unique ways and as the number of chronic conditions increases, so do the challenges to managing health care.
